All Registration Packets must include a signed Physical form.  You can download the CHSAA Medical eligibility form to take to your doctor's appointment to ensure we receive the correct documentation for your student.  Payment must be received before the athlete is allowed to participate in practices. 

Please go to http://payforit.net to pay all Athletic Fees.  A fee will be billed to your student's account after we have received your registration form.
