Athletics Registration Form:

All Registration Packets must include a signed Physical form. You can download the CHSAA Medical eligibility form to take to your doctor's appointment to ensure we receive the correct documentation for your student. Payment must be received before the athlete is allowed to participate in practices. Please click on your preferred link to register your student:

Registration is currently OPEN for CoEd Cross Country and Girls Soccer. Registration will close on 8/21/2026 at midnight!

Athletics Fees

Please visit our new fee platform, PaySchools to pay all Athletic Fees. A fee will be billed to your student's account after we have received your registration form. Each sport costs $60. Students who sign up for 3 or more different sports will only have to pay the fee for the 1st two sports (maximum charge of $120 per student). Free and reduced benefits will apply towards your student's sport fees.
